The information contained in this application is correct to the best of my knowledge. I give GracePointe Wesleyan Church permission to complete a background check on me. I hereby authorize any agency, individual or church listed in this application to provide GracePointe Wesleyan Church with any information they may have regarding my character and fitness to work with children and youth.I release all such agencies, individuals or churches (including GracePointe Wesleyan Church) from liability for any damage that may result from furnishing such evaluations. I further waive any right to inspect the references provided on my behalf. Should be application be accepted, I agree to be bound by the constitution and bylaws and policies of GracePointe Wesleyan Church and to refrain from unscriptural conduct.
